PT - Physical Therapist Job Details: • Work 8-hour day shifts from 8:00 AM to 5:00 PMOpportunity:PT - Physical TherapistJob Details:• Work 8-hour day shifts from 8:00 AM to 5:00 PM• 5 days per week, full-time schedule• Weekend requirement: Friday through Tuesday shift schedule• No on-call requirement• Floating requirement to Inpatient Rehab/Acute settings• Uniform top is issued as part of dress code• Current New Mexico Physical Therapist licensure required• BLS certification from AHA required• Bachelor's, Master's, or entry-level Doctorate of Physical Therapy degree from an accredited college or university required• Proficiency with all modalities and equipment used by a Physical Therapist required• Perform comprehensive diagnostic evaluations to determine physical and cognitive status of inpatients and outpatients• Develop and implement individualized treatment plans with measurable goals in collaboration with patients and families• Maintain thorough documentation including initial patient evaluations, daily and weekly progress notes, and discharge summaries• Participate in patient care conferences, family conferences, in-service programs, and departmental meetings• Counsel patients, families, and other involved individuals regarding evaluation, care, and treatment• Initiate and complete discharge planning in a timely manner and provide effective education and training• Assist with coordination and supervision of Physical Therapy and Physical Therapy Assistant student programs• Assist with orientation of new associates and scheduling of patients to ensure quality coverage• Participate in Performance Improvement activities and maintain departmental productivity standards• Maintain awareness of community agencies to make appropriate referrals and increase knowledge of community resources• Demonstrate competence in age-specific, developmental, and population-specific patient regional medical center is a full-service healthcare facility offering a broad range of inpatient and outpatient services, including a dedicated rehabilitation department.
